Evergreen Health Services

In 1996, ACS applied for and received an Article 28 License from the New York State Department of Health to provide primary medical services with an HIV specialty.  This was part of the agency’s strategic plan to provide a comprehensive continuum of services to families, individuals and communities affected by the HIV/AIDS epidemic.

PRIMARY CARE: The primary mission of Evergreen Health Services is to provide a comprehensive network of care, which is sensitive to the diverse needs of the people we serve. We provide the same services as a family doctor would, with a specialty in the care of persons living with HIV/AIDS and a sub-specialty in Gay, Lesbian, Bisexual, and Transgendered health care.

To assist you in obtaining your health needs, our medical staffing includes our Board-Certified Internal Medicine and HIV specialist M.D.; a Nurse Practitioner, a Registered Nurse/Adherance Counselor, support clinical staff such as LPNs and a Patient Services Specialist, and a Registered Dietician. Additionally, our clerical and administrative staff are available to assist with your non-medical needs. One on one Prescription Adherence counseling is available for all HIV+ patients.

To contact the office please call 847-0328 between the hours of 8:30 a.m. and 3:30 p.m. Monday through Friday. Evergreen Health Services is open from 8:30 am to 4:00 pm Monday through Friday. If you need to reach your health care provider after hours, you may call our answering service at 879-0168.

HIV TESTING: Knowing your HIV status is an important part of preventing HIV and ending the AIDS epidemic. AIDS Community Services now offers the free and 100% confidential HIV Antibody (AIDS) test at all its service sites. Please Click here for service site locations and contact information.

TREATMENT ADHERENCE: Adherence to HIV treatment is the key to keeping the HIV virus in check.  Our goal is to achieve undetectable virus levels in your blood.  This will keep you healthier longer and prevent complications of HIV.

At Evergreen Health Services an important part of care is evaluating adherence to treatment.  Lab testing of T-cell counts and measurement of the amount of virus in your body is done on a regular basis to monitor your progress.  This information is then evaluated by our Medical Director.  Special testing to check for resistance to medications can be done.  Patients are evaluated on an individual basis at each doctor's visit and problems such as side effects, amounts of pills taken, and privacy issues are addressed.  Individual appointments for adherence are arranged to assist patients with any challenges they may be facing.
