Case Manager
The Case Manager is responsible for working directly with individuals and families with HIV/AIDS and developing service plans needed to coordinate a variety of needs including medical care, housing, entitlements, mental health care, and substance abuse.Bachelor’s degree in health, education or human services (preferred) or RN AND one year of case management or case work with persons with HIV/AIDS, a history of mental illness, homelessness, chemical dependency, and/or other populations of persons in need.
OR Associates degree in health, education or human services (preferred) or LPN or Certification as a CASAC AND two years of case management or case work with persons with HIV/AIDS, a history of mental illness, homelessness, chemical dependency, and/or other populations of persons in need.
OR 60 credit hours of college study from a regionally accredited college or university or one recognized by the NYS Education Department as following acceptable educational practices AND two years of case management or case work with persons with HIV/AIDS, a history of mental illness, homelessness, chemical dependency, and/or other populations of persons in need. Fluency in English and Spanish a plus. Sensitivity to AIDS issues essential. Must have own vehicle and work flexible hours
Nutrition Program Assistant
The Nutrition Program Assistant provides clerical, organizational and client contact support for the department. Associates degree in Health or Human Services required; Bachelor’s degree preferred. One year experience working in the field of chronic illness or HIV/AIDS. Must demonstrate ability to communicate clearly and professionally; sensitivity to cultural, HIV/AIDS and LGBT issues essential. Ability to lift 50 pounds without restrictions. Fluency in English and Spanish a plus.
Outreach Specialist – Pride Center
The Outreach Specialist for the Pride Center of WNY is responsible to recruit clients through outreach, conduct client intakes and make referrals to testing. Associates degree in Human Services or related field required; Bachelor’s degree preferred. Two years experience working with the L/G/B/T communities required; sensitivity to HIV/AIDS and lifestyle issues is essential. Must have a vehicle, be willing to travel and work flexible hours.
